A minimum of twenty hours of Community Service.

For each individual or agency where Community Service was performed, a summary must be submitted. In the summary the following points must be addressed.

Description of the activity performed.

Impact that this service has on your community.

Signature from Supervisor or person you completed the Community Service for.

When this is complete, please present to your advisory teacher.
